A very warm welcome to you!

SALT Fellowship is committed to providing a safe and caring environment to all our participants, regardless of gender, ethnicity, religion or disability.  Our Code of Conduct for participants summaries our expectations for our participants and its consequences for unacceptable behaviour.  Please click [here] for the Code of Conduct.

 

If you are joining us for the first time, you are required to do the following BEFORE your participation in our activities:

(1) Submission of the Letter of Declaration and Consent (LDC). 

(2) Payment of a non-refundable [administrative fee] of S$10 

You are only required to submit LDC and pay an administrative fee ONCE to become a participant in SALT programmes, events and activities.  For example, you may join in the many different walks organized under SALT-CARE.  However, you may have to pay extra to participate in certain activities, for example, you have to pay for the costs of materials in Flower Arrangement, Fruit Art, cooking demonstrations, etc.

Payment of the administrative fee (non-refundable) of S$10 via three ways:

After you have transferred the funds, please email our Honorary Treasurer, treasurer@saltministry.sg on the same day, stating your FULL Name and Mobile No. An e-receipt will be issued upon request.

To: SALT FELLOWSHIP MANAGEMENT COMMITTEE

 

I declare that I shall be participating in the events and activities organised by SALT Fellowship. I understand that while reasonable precautions will be taken by SALT Fellowship and/or its office-bearers, staff, volunteers, and other members of the organising committee (collectively, the “Agents”) to ensure the safety of participants (also known as SALTers), I acknowledge and accept that I participate in such events and activities entirely at my own risk.

I undertake to seek appropriate medical advice or clearance prior to participating in any SALT Fellowship event or activity if I have any pre-existing medical or health conditions. I further agree to obtain and maintain my own Personal & Accident Insurance before participating in any SALT Fellowship events or activities that may involve strenuous physical exertion.

I hereby agree and undertake not to hold liable, nor to make any claim against, SALT Fellowship and its Agents for any loss of or damage to my property, or for any injury, illness, or death suffered by me in connection with such events or activities, whether I participate as a participant or a volunteer, and whether such events or activities take place within or outside SALT Fellowship’s premises.

I understand and consent to the collection, use, disclosure, and storage of my personal data by SALT Fellowship for purposes related to communication, event organisation, and administration. I further consent to my personal data being disclosed to, stored, and used by SALT Fellowship’s partners, service providers, and sponsors in connection with the organisation of such events and activities. I undertake to notify SALT Fellowship’s Data Protection Officer of any changes to my personal data or if I wish to withdraw my consent, and I acknowledge that I may contact the Data Protection Officer should I require any clarification regarding the protection of my personal data.

I also consent to the use of my image and/or voice in photographs, audio recordings, and video recordings taken during SALT Fellowship events or activities for SALT Fellowship’s publications, publicity, and promotional purposes across all forms of media. I acknowledge and agree that all such recordings, articles, quotations, photographs, films, audio or video recordings, and any reproductions thereof shall remain the property of SALT Fellowship.

This Letter of Declaration and Consent shall be binding upon me, my estate, and my legal representatives.

SALT Fellowship undertakes that all personal data collected shall be protected in accordance with the relevant provisions of the Personal Data Protection Act.

Upon submission of LDC form and payment, we will usually take up to 2 weeks processing time before you are added to the main SALT Community WhatsApp group/issue of e-receipts.
